A Person Centred Service

At Home From Home Care, we adapt our services to meet the user's needs, hopes and aspirations.

Everybody will have a person centred plan that reflects the individual's choices, dreams and aspirations, and each individual's plan will have meaningful and achievable goals.

This person centred plan will be developed following an in-depth assessment, in partnership with the individual, their carers and the referring and purchasing bodies.
