Domiciliary Care

HFHC is currently registering to become a Domiciliary Care provider, offering Community Supported Living.

This new service will be available early in 2010, enabling us to support people living in the community, either with their families or in their own homes.

We will offer support with personal care, as well as help and support for each person to learn new skills, develop independence, access other services and enjoy life as part of the community.

We will provide home based support to individuals who are referred by Social Services or who are puchasing services through Direct Payments or Individual Budgets.

Each person will have a Support Plan detailing the support they will receive and an Activity Planner detailing their daily activities.

Information will include personal care, learning opportunities, recreational, social, respite breaks, vocational and work activities.

It may also include support to access community facilities such as college, work or leisure.
