Assessments

The starting point to a successful outcome at Home From Home is our comprehensive needs led assessment, which follows a referral and ensures we understand each individual's needs and aspirations.

Following the assessment, we work in close partnership with the individual, their care manager and their families or other representatives, to develop a Service Delivery Plan.

This sets out their support needs and details how they will be met, and matches them to the appropriate Home From Home Care service, thus ensuring we can meet the individual's needs.
