Referrals

• Through the individuals Social Services Department Care Management Team or, where relevant, their Health Authority
  

• By the individual (or someone acting on their behalf) who wishes to use their Direct Payment or Individual Budget, or who is self-financing.

Where an individual is seeking help to pay for their placement, the approach must be made in consultation with Social Services or the Health Authority, following their Care Management procedures.
